These terms apply between you and (org.nr. ), which provides Formwren.
Formwren is only for licensed health professionals working with aesthetic or cosmetic treatments. You must give a valid professional registration number, and we must have verified it, before you can use the app. Your account is personal: don't share your password, and don't let clients use the clinician review.
Formwren is a screening aid. It does not diagnose any condition and does not replace your clinical judgement. You are responsible for every treatment decision, referral and conversation with your client. We review the point values regularly against new research and feedback from clinicians.
Client answers stay on your device and are never sent to us. You are responsible for informing your clients, getting their consent where needed, and for any copy you keep in your own records, under the rules that apply to your clinic.
Set a clinician PIN before handing the device to a client, and keep the device in your sight. Sign out on devices you no longer control.
We can suspend or remove access if a registration is no longer valid, if the terms are broken, or to protect the service. You can delete your account at any time in the app.
The app is provided as it is. To the extent the law allows, we are not liable for indirect losses or for decisions made using the app. Nothing in these terms limits rights you cannot give up under the law.
Norwegian law applies. Disputes go to the courts of the company's registered office, unless mandatory rules say otherwise.
