Know who you can help before the consultation starts.

A pre-treatment screening for aesthetic clinics. Clients answer the questionnaire and confirm the consent form on a tablet in the waiting room, in English or Spanish. You see the warning signs of obsessive body focus and other mental-health risks before they sit down. The decision stays with you.

For verified clinicians only. Free to use. Client answers are never stored.

“But I can't live like this. I'll pay anything.”A client at our clinic, asking for a second hair transplant

The result was a success. She couldn't see it.

Four years earlier, her transplant had gone exactly to plan. In the mirror she saw an uneven hairline and thin hair. As the consultation went on, more came out: lips, cheeks, Botox, laser, weight-loss injections, and a question about fillers.

We said no, for the right medical reasons. But by then she had shown eight warning signs, and she left with a no and nothing else.

The right questions on the registration form would have shown most of this before she walked in. That's why we built Formwren.

Details changed to protect the client.

How it works

It fits into the registration you already do, and adds about eight minutes in the waiting room.

  1. The client answers on a tablet

The form is titled “Pre-treatment questionnaire”. Clients never see a score, and they hand the tablet back when they're done.

  2. You see the result before you meet them

Unlock with your clinician PIN. Fifteen areas light up green, yellow or red, and every yellow or red area comes with questions to ask in the consultation.

  3. You decide and explain

Go ahead, or explain why treatment isn't right now with a script built from the client's own answers. Formwren writes the record summary and the GP letter.

What it screens for

Studies in cosmetic and dermatology settings estimate that roughly 5 to 15 percent of clients show signs of body dysmorphic disorder. For most of them, treatment doesn't ease the distress. The test also looks for other conditions that should pause treatment.

Obsessive body focus

Time spent on the concern, checking and hiding, avoiding people and places, how many areas, and whether others see the flaw.

Treatment history

How many treatments, how satisfied the client was, and repeat requests for the same area.

Expectations

Hoping treatment will change their life, wanting to look like someone else, and how they'd cope with an imperfect result.

Mental health stop or assess

Thoughts of self-harm, eating disorders (SCOFF), low mood (PHQ-2), anxiety (GAD-2), psychotic experiences and substance use.

What you notice in the room

Ten observations, from fixating on details you can't see to “I can't live like this”, each with a clear next step.

Consent and records

A 12-part informed consent the client confirms on the tablet, a summary for the client record, and a concern letter to the GP.

Built around the clinician

You decide

The test suggests a next step. The clinician or clinic manager makes the call.

Nothing is stored

The free version runs in the browser. Answers are erased when you start a new client.

Not a diagnosis

It shows who needs a closer look, and points them to a doctor or psychologist.

Discreet for clients

Clients see a standard questionnaire in English or Spanish. Scores appear only after a clinician unlocks the review with a PIN.

Questions

Is this a diagnostic test?

No. It's a screening aid for trained clinicians. It flags clients who need a closer look and suggests a next step, such as a referral to their GP or a psychologist. It does not diagnose body dysmorphic disorder or any other condition.

Where are the answers stored?

In the free version, nowhere. The test runs in your browser, and answers disappear when you start a new client or close the page. Copy the summary into your own client record if you want to keep it.

What is it based on?

The questions draw on clinical research on body dysmorphic disorder in cosmetic settings, plus established short screens: the SCOFF eating questions, PHQ-2 for low mood and GAD-2 for anxiety. We review the point values regularly against new research and feedback from clinicians.

Will clients feel judged?

The client sees a standard pre-treatment questionnaire that explains why the questions are asked. They never see a score or the name of the test.

Which languages does it support?

The whole app is available in English and Spanish: the client questionnaire, the consent form, the clinician review, the talking script, the record summary and the GP letter. Switch language at any time from the top of the screen. More languages are planned.

Who can use Formwren?

Only licensed health professionals. You request access with your professional registration number (in Norway, your HPR number). We check it in the official register before the app opens. This stops clients from installing the app to rehearse or change their answers.

What do you store about me?

Only what we need to verify you: your name, work email, registration number, country and clinic. Nothing about your clients. You can delete your account at any time from the app. Read the privacy notice.

What equipment do I need?

Any tablet, phone or computer with a web browser. You can also install it from the App Store or Google Play, or add it to your home screen.
